Deze verklaring legt uit welke persoonsgegevens de praktijk verwerkt, waarom, hoe lang, en welke rechten u daarbij heeft. Ze geldt zowel voor deze website als voor de zorg in de praktijk.

Deze website
De website zet geen cookies en gebruikt geen analyse-, advertentie- of trackingsoftware.
Er staat één formulier op, waarmee u de praktijk een bericht kunt sturen. Wat u invult — uw naam, e-mailadres, eventueel uw telefoonnummer en uw bericht — wordt per e-mail naar [email protected] gestuurd. U hoeft het formulier niet te gebruiken: bellen of rechtstreeks mailen kan altijd.
De verzending verloopt via Formspree (Formspree Inc., Verenigde Staten), dat het bericht doorstuurt en een kopie bewaart in de account van de praktijk. Zij treden op als verwerker en gebruiken uw gegevens niet voor eigen doeleinden. Doorgifte buiten de EU gebeurt op grond van de standaardcontractbepalingen van de Europese Commissie.
Berichten uit het formulier worden niet langer bewaard dan nodig is om u te antwoorden, en daarna verwijderd. Gaat het bericht over een behandeling, dan kan het in uw patiëntendossier terechtkomen; dan gelden de bewaartermijnen hieronder. Stuur geen medische gegevens via het formulier — bespreek die op de raadpleging.
Twee technische voorkeuren worden lokaal in uw browser bewaard. Ze blijven op uw toestel, worden nooit naar ons verzonden en zijn voor ons niet leesbaar. Het cookiebeleid beschrijft ze in detail.
De lettertypen worden vanaf deze website zelf geladen. Om een pagina te tonen wordt geen enkel extern domein aangesproken.
Hosting
De website wordt gehost door Cloudflare, Inc. Zoals elke webserver houdt die logbestanden bij, met onder meer het IP-adres, het tijdstip en de opgevraagde pagina. Dat gebeurt voor beveiliging en storingsopsporing, op grond van ons gerechtvaardigd belang om de site werkend en veilig te houden.
Links naar derden
De website verwijst naar Doctena om een afspraak te maken, naar Google Maps voor de route, naar de patiëntenpagina van ClearCorrect bij Straumann en naar OrthoApnea. Volgt u zo'n link, dan gelden vanaf dat moment de regels van die partij. Maakt u een afspraak via Doctena, dan verwerkt Doctena uw gegevens als eigen verantwoordelijke — niet deze website.
Gegevens in de praktijk
Voor uw behandeling houdt de praktijk een patiëntendossier bij. Daarin komen:
- identiteits- en contactgegevens, rijksregisternummer en ziekenfonds;
- medische en tandheelkundige gegevens: anamnese, diagnose, behandelplan en verloop, voorgeschreven geneesmiddelen;
- beeldmateriaal: röntgenopnamen, scans en klinische foto's;
- afspraak-, facturatie- en betalingsgegevens.
Waarom, en op welke grondslag
- zorgverlening en het bijhouden van het dossier — artikel 9, lid 2, h) AVG, samen met de wettelijke verplichting een patiëntendossier bij te houden;
- afspraken maken en met u communiceren — uitvoering van de overeenkomst;
- facturatie, derdebetalersregeling en boekhouding — wettelijke verplichting.
Wie de gegevens ontvangt
- medewerkers van de praktijk, elk binnen hun taak en gebonden door het beroepsgeheim;
- tandtechnische laboratoria, voor het vervaardigen van uw toestel of prothese;
- verwijzende of mee behandelende zorgverleners en ziekenhuizen, waaronder CHIREC en UZ Brussel, voor zover dat voor uw zorg nodig is;
- uw ziekenfonds en het RIZIV, voor terugbetaling;
- de boekhouder, voor de facturatie.
Uw gegevens worden niet verkocht, niet verhuurd en nooit voor reclame gebruikt.
Hoe lang
Het patiëntendossier wordt dertig jaar na het laatste contact bewaard, zoals de wet voorschrijft. Facturatie- en boekhoudkundige stukken worden zeven jaar bewaard.
Buiten de Europese Economische Ruimte
Patiëntgegevens worden niet buiten de EER doorgegeven.
Uw rechten
U heeft het recht op inzage in uw gegevens en op een kopie ervan, op verbetering van wat onjuist is, op wissing, op beperking van de verwerking, op bezwaar en op overdraagbaarheid.
Voor een patiëntendossier gelden grenzen: zolang de wettelijke bewaartermijn loopt, kan een dossier niet zomaar worden gewist. U heeft wel altijd recht op inzage en op een afschrift, en u kunt een vertrouwenspersoon aanwijzen.
Een verzoek stuurt u naar [email protected]. We vragen u zich te identificeren, zodat gegevens niet bij de verkeerde persoon terechtkomen, en antwoorden binnen een maand.
Klacht
Bent u niet tevreden over hoe de praktijk met uw gegevens omgaat, laat het ons dan eerst weten. U kunt ook klacht indienen bij de Gegevensbeschermingsautoriteit, Drukpersstraat 35, 1000 Brussel — [email protected].

Le site ne dépose aucun cookie et n'utilise ni analyse d'audience, ni publicité, ni traçage.
Il comporte un formulaire, qui permet d'envoyer un message au cabinet. Ce que vous y inscrivez — votre nom, votre adresse e-mail, éventuellement votre numéro de téléphone et votre message — est transmis par e-mail à [email protected]. Son usage n'est pas obligatoire : vous pouvez toujours téléphoner ou écrire directement.
L'envoi passe par Formspree (Formspree Inc., États-Unis), qui transmet le message et en conserve une copie dans le compte du cabinet. Formspree agit comme sous-traitant et n'utilise pas vos données à ses propres fins. Le transfert hors UE repose sur les clauses contractuelles types de la Commission européenne.
Les messages reçus via le formulaire ne sont pas conservés plus longtemps que nécessaire pour vous répondre, puis sont supprimés. Si le message porte sur un traitement, il peut rejoindre votre dossier patient : les délais de conservation ci-dessous s'appliquent alors. N'envoyez pas de données médicales via le formulaire — parlons-en en consultation.
Deux préférences techniques sont conservées localement dans votre navigateur. Elles restent sur votre appareil, ne nous sont jamais transmises et ne nous sont pas lisibles. La politique de cookies les détaille.
Les polices de caractères sont servies par ce site. Aucun domaine externe n'est sollicité pour afficher une page.
Hébergement
Le site est hébergé par Cloudflare, Inc. Comme tout serveur web, celui-ci conserve des journaux comprenant notamment l'adresse IP, l'horodatage et la page demandée, à des fins de sécurité et de diagnostic, sur la base de notre intérêt légitime à maintenir le site en état de marche.
Liens vers des tiers
Le site renvoie vers Doctena pour la prise de rendez-vous, vers Google Maps pour l'itinéraire, vers la page patients de ClearCorrect chez Straumann et vers OrthoApnea. Dès que vous suivez un de ces liens, les règles de ces tiers s'appliquent. Si vous prenez rendez-vous via Doctena, c'est Doctena qui traite vos données en tant que responsable — pas ce site.
Les données au cabinet
Pour vos soins, le cabinet tient un dossier patient. Il contient :
- données d'identité et de contact, numéro de registre national et mutualité ;
- données médicales et dentaires : anamnèse, diagnostic, plan de traitement et son déroulement, médicaments prescrits ;
- imagerie : radiographies, scans et photographies cliniques ;
- données de rendez-vous, de facturation et de paiement.
Finalités et bases légales
- les soins et la tenue du dossier — article 9, §2, h) du RGPD, avec l'obligation légale de tenir un dossier patient ;
- la prise de rendez-vous et la communication — exécution du contrat ;
- la facturation, le tiers payant et la comptabilité — obligation légale.
Destinataires
- les collaborateurs du cabinet, chacun dans le cadre de sa mission et tenus au secret professionnel ;
- les laboratoires de prothèse dentaire, pour la fabrication de votre appareil ;
- les praticiens et hôpitaux qui vous adressent ou vous co-traitent, dont le CHIREC et l'UZ Brussel, dans la mesure nécessaire à vos soins ;
- votre mutualité et l'INAMI, pour le remboursement ;
- le comptable, pour la facturation.
Vos données ne sont ni vendues, ni louées, ni utilisées à des fins publicitaires.
Durées de conservation
Le dossier patient est conservé trente ans après le dernier contact, conformément à la loi. Les pièces de facturation et de comptabilité sont conservées sept ans.
En dehors de l'Espace économique européen
Les données de patients ne sont pas transférées hors EEE.
Vos droits
Vous avez le droit d'accéder à vos données et d'en obtenir copie, de les faire rectifier, de demander leur effacement, la limitation du traitement, de vous y opposer et à la portabilité.
Le dossier patient connaît des limites : tant que court le délai légal de conservation, il ne peut être simplement effacé. Vous gardez en tout temps le droit de le consulter et d'en recevoir copie, et vous pouvez désigner une personne de confiance.
Adressez votre demande à [email protected]. Nous vous demanderons de vous identifier, afin qu'aucune donnée ne parvienne à la mauvaise personne, et répondrons dans le mois.
Réclamation
Si la manière dont le cabinet traite vos données ne vous satisfait pas, dites-le nous d'abord. Vous pouvez également introduire une réclamation auprès de l'Autorité de protection des données, rue de la Presse 35, 1000 Bruxelles — [email protected].

The website sets no cookies and uses no analytics, advertising or tracking software.
It carries one form, for sending the practice a message. What you put in it — your name, your email address, your telephone number if you give one, and your message — is sent by email to [email protected]. You do not have to use it: telephoning or writing directly is always open to you.
Sending goes through Formspree (Formspree Inc., United States), which forwards the message and keeps a copy in the practice's account. They act as a processor and do not use your data for their own purposes. Transfer outside the EU rests on the European Commission's standard contractual clauses.
Messages from the form are kept no longer than answering you requires, and are then deleted. If a message concerns a treatment it may join your patient record, in which case the retention periods below apply. Please do not send medical details through the form — bring them to a consultation.
Two technical preferences are kept locally in your browser. They stay on your device, are never sent to us and cannot be read by us. The cookie policy describes them in full.
Typefaces are served from this website. No external domain is contacted in order to render a page.
Hosting
The site is hosted by Cloudflare, Inc. Like any web server it keeps logs, including IP address, timestamp and the page requested, for security and fault-finding, on the basis of our legitimate interest in keeping the site working and safe.
Links to third parties
The site links to Doctena for appointments, to Google Maps for directions, to ClearCorrect's patient page at Straumann and to OrthoApnea. Once you follow such a link, that party's rules apply. If you book through Doctena, Doctena processes your data as its own controller — not this website.
Data at the practice
For your treatment the practice keeps a patient record. It contains:
- identity and contact details, national register number and health insurance fund;
- medical and dental data: history, diagnosis, treatment plan and its course, prescribed medication;
- imaging: radiographs, scans and clinical photographs;
- appointment, invoicing and payment data.
Purposes and legal bases
- providing care and keeping the record — Article 9(2)(h) GDPR, together with the legal duty to keep a patient record;
- arranging appointments and communicating with you — performance of the contract;
- invoicing, third-party payer arrangements and accounting — legal obligation.
Who receives the data
- staff of the practice, each within their role and bound by professional secrecy;
- dental laboratories, to make your appliance or prosthesis;
- referring or co-treating practitioners and hospitals, including CHIREC and UZ Brussel, so far as your care requires;
- your health insurance fund and the NIHDI, for reimbursement;
- the accountant, for invoicing.
Your data is never sold, rented or used for advertising.
How long
The patient record is kept for thirty years after the last contact, as the law requires. Invoicing and accounting records are kept for seven years.
Outside the European Economic Area
Patient data is not transferred outside the EEA.
Your rights
You have the right to see your data and receive a copy, to have what is wrong corrected, to ask for erasure, for restriction of processing, to object, and to data portability.
A patient record has limits: while the statutory retention period runs, a record cannot simply be erased. You always keep the right to consult it and to receive a copy, and you may appoint a person of trust.
Send your request to [email protected]. We will ask you to identify yourself, so that data never reaches the wrong person, and will answer within one month.
Complaints
If you are unhappy with how the practice handles your data, tell us first. You may also complain to the Belgian Data Protection Authority, Drukpersstraat 35, 1000 Brussels — [email protected].
